Required documents

In addition to providing proof of education and entry-to-practice examination documents, registrants must provide the following required documents to the College in an electronic format within their account in the online registration system - HMS (file types accepted: .jpg, .png, or .pdf, not larger than 8 MB).

Criminal record screening

The criminal record screening is a requirement for provisional and general registration. The Registrar may also request a criminal record screening from any registered member randomly selected for an audit if the original document is no longer valid at the time of the audit. The criminal record screening results document is considered valid for five years.

Criminal record screening results must include a vulnerable sector check.

If the report indicates a criminal finding, applicants must submit a detailed explanation of the circumstances that led to the criminal finding. Applicants may be asked to provide a copy of charging documents, court transcripts and any relevant documentation related to the matter.

The report must include records of discharges that have not been removed from all databases in accordance with the Criminal Records Act and records of outstanding criminal charges of which the RCMP is aware.

The full name appearing on the vulnerable sector check must match the name used to register for the College. The report must also indicate that a search was conducted under all previous legal names used by the registrant.

Certificate of insurance (renewed annually)

The certificate of insurance must verify that you are covered by a current and valid professional liability insurance (PLI) with the minimum coverage amount of $5 million per occurrence and $5 million aggregate per year. Insurance payment receipts do not verify coverage and are not accepted.
